Answer 1:

I don't know when it was that people realized that air becomes less dense as you go up in altitude, or realized that there was vacuum up above. I imagine that was something that came about fairly slowly, although I'm pretty sure it was well-known by Newton's day (otherwise, there would be friction, as Newton himself would have recognized).
